OmegaT doesn't work inside MS Word but it does open DOCX files directly, without any conversion to an intermediary format. It presents the translatable text as tagged text, similar to what you'd find in tools that do use such formats.

Samuel, thanks so much for the helpful info and the Felix installation suite! I have loads of FTMs so that is a huge help!
